Cost is $25 if we complete it for you.
This form must also be completed and printed out as well as the first. The cost is free for this form: https://fortress.wa.gov/dshs/bcs/.
Teberculosis Test
Required for all medical programs
A 2 step T. B. Test OR QuantiFERON-TB Gold OR Chest X-RAY (must be one year old or less)
